To order by fax, print out our order form in either (.PDF format) or (.DOC format) and include your credit card information. Checks (No more than $200.00) and money orders (U.S Funds Only) must be mailed to us, along with your printed out order form, and sent to the address at the bottom of this page. ALL payment information must be filled out for credit cards or your payment will be rejected and your request for an order will be canceled. Simply fax the completed order form to (480) 357-6060. Please include your e mail address AND telephone number on the order form, just in case we need to ask you any questions. Your receipt will be sent to the billing address that you provide.
